Million Hearts Recognition and CDPHE funding applications due

Health professionals, practices, and health systems that have achieved hypertension control rates of at least 80% are eligible to enter the 2019 Million Hearts Hypertension Control Challenge. Colorado congratulates the following 2018 Champions:

David Yamamoto, MD, Peak to Peak Family Medicine in Arvada; High Plains Community Health Center Holly Clinic in Holly; Hinman Family Medicine in Longmont and Southern Colorado Medicine in Colorado Springs.

If your practice has achieved a blood pressure control rate of at least 80 percent for patients with hypertension, you can join this stellar group of providers by submitting an application to be a Million Hearts 2019 Hypertension Control Champion. Applications are due April 1.

Not at 80% yet? We can help! CDPHE's Request for Applications #11460 is a combined effort from multiple chronic disease prevention and management programs at the Colorado Department of Public Health and Environment to promote and improve chronic disease and health outcomes among underserved Coloradans. This funding opportunity incorporates many of the services provided through the WISEWOMAN program, the Clinic Quality Improvement for Population Health Initiative and the Diabetes and Cardiovascular Disease Prevention and Management program. Applications are due March 31.
