Grants for Prediabetes and Hypertension in Priority Populations

Building Capacity of Providers to Address Prediabetes Among Disparate Populations is a project for providers and health care systems to screen, test and refer African American and Hispanic women. The deadline for applications is March 19th. The application can be found here. Funding is for $25,000 per year, with $8,500 for the start-up period.

Reducing Hypertension Among Priority Populations is aimed at helping providers and health care systems to increase identification and referral of African American men ages 35-64 to control hypertension. The application deadline is March 30th and the Call for Proposals may be found here. Funding is for three years, $15,000 per year, with $5,000 start-up.
