Colorado Primary Care Collaborative

House Bill 1365, a bipartisan bill, establishes a Primary Care Payment Reform Collaborative. This Collaborative will be comprised of representatives from all sectors of the healthcare industry, who will be tasked with generating an annual report on the investments currently being made in primary care, as well as trends in the implementation of Alternative Payment Models that shift toward value-based payment. 


This Payment Reform Collaborative will also look at how to expand the adoption of Alternative Payment Models, including ways to align payment models and build upon efforts of the State Innovation Model and CPC+ to align quality measures.
