CDC Seeking Multi-Site Health Systems for Hypertension Management Program

The Centers for Disease Control and Prevention's (CDC) is committed to disseminating evidence-based practices to improve hypertension control among priority populations, including uninsured, under-insured, and the medically underserved. As such, CDC is seeking nominations of clinics or health systems to serve as a replication site for an evidence-based Hypertension Management Program and to evaluate the impact of the program. The program is a team-based, patient-centered, integrated care model with demonstrated effectiveness at improving blood pressure control among participants.
