Community Guide Recommends CHW Interventions for Diabetes and CVD Management

The Community Preventive Services Task Force (CPSTF) recommends engaging community health workers (CHWs) in diabetes and cardiovascular disease management based on strong evidence of effectiveness in improving blood glucose level, lipid control and blood pressure and improving appropriate use of healthcare services.

CHWs can provide culturally appropriate patient education, coaching, informal counseling and social support, medication adherence, diet, physical activity or weight management support, connections to services and in some cases delivering health services such as blood pressure screening. Through these activities, CHWs provide support to improve diabetes and cardiovascular disease care and self-management behaviors among patients.
