HealthSlate launching new hybrid DPP program

HealthSlate is now offering a new in person/digital Diabetes Prevention Program (DPP) and is seeking partnership in the Denver area for the first pilot cohort, at no cost to the organization or participant.  

This particular "hybrid" program gives participants the best of both worlds, i.e. the emotional support from attending in-person when that works for them, and the flexibility of participating virtually when that works better. All materials and coaching would be provided by a trained Lifestyle Coach from HealthSlate at your location. Partners would just need to provide participant referrals and a location for classes to be held.  

The HealthSlate app includes CDC-approved videos/modules for each session of the PreventT2 or 2012 curriculum, HIPAA compliant messaging, a Group Wall for collaboration, fat gram/calorie goal budget, a food database with extensive nutritional information for over 4000 foods, and the ability to sync with Fitbit, Apple health or Google Fit, plus many more features.
