New Guide for Pharmacists on Reducing Heart Disease and Stroke in the U.S.

Community pharmacists have a key role to play in improving population health. CDC developed this publication, Using the Pharmacists’ Patient Care Process to Manage High Blood Pressure: A Resource Guide for Pharmacists, as a call to action to use the Pharmacists’ Patient Care Process as a way to  prevent and manage high blood pressure through team-based care, with the goal of reducing heart disease and stroke in the United States.

The guide compiles information about current resources and emerging practices, as well as tools and examples that pharmacists can use to help them improve health outcomes associated with cardiovascular disease.
