Funding Opportunity: Tele-health
As part of the Foundation's strategy to strengthen the delivery of comprehensive, person-centered primary care, this initiative will build capacity in primary care clinics for the implementation of tele-health services. Funding will support up to $100,000 for costs associated with the purchase of tele-health equipment and/or the employment of workflow or billing consultants. Private practices may also apply for low interest loans. The Foundation seeks applications that focus on:
- Teleconferencing between providers and patients
- Telemonitoring to increase regular contact with care providers and monitor conditions
- Other innovative telehealth programs that increase access to comprehensive, coordinated primary care services through health-related technology
