Jefferson County Public Health has created the Jeffco Community Health Improvement Network, with more than 100 community partners working together to increase physical activity, healthy eating, and psycho-social well-being in Jefferson County. The Network was formed based on goals outlined in the Jefferson County Community Health Improvement Plan (CHIP).
The Jeffco Community Health Improvement Network is an umbrella organization for six coalitions using the collective impact framework to improve the health of our community. - Active Living Coalition
- Food Policy Council/Coalition
- Healthy Beverage CoalitionHealth in Early Childhood Collaborative
- School Wellness Coalition
- Preventive Care and Mental Health Resources Coalition is under development and will focus on establishing methods and measures for the coordination of the provision of preventive care and mental health care.
