Colorado SIM Office Announces Four Bi-Directional Integrated Health Home Sites

In partnership with the Colorado Behavioral Healthcare Council, the Colorado State Innovation Model office announces four sites selected for the Bi-Directional Integration Demonstration and Practice-Based Research Pilot Program within the larger Colorado SIM initiative.
  • Community Reach Center, Commerce City 
  • Jefferson Center for Mental Health, Lakewood 
  • Mental Health Partners, Boulder 
  • Southeast Health Group, La Junta 
Over the next four years, these sites will pilot models of bi-directional integrated care and practice-based research. Bi-directional integration refers to the integration of primary care and prevention services into a community behavioral health setting. 
This was a highly competitive application process that clearly demonstrated the strength, innovation, and dedication of Colorado’s behavioral health network. 

Under the strategic leadership of CBHC and the Colorado SIM Office, these sites will create alignment within the broader Colorado SIM initiative and gather health outcome, quality, cost, and payment data to inform the design of value-based payment systems for high quality integrated care. The sites will participate in a learning collaborative with the other awardees and a robust network of stakeholders to share learnings, outcomes, and best practices throughout the pilot program. The lessons learned from these sites will be shared and leveraged to create lasting, transformational change.
