EvidenceNOW Southwest: Advance your practice while improving heart health

Independent primary care practices with 10 or fewer providers are eligible to engage in a national program that offers in-office practice facilitation, participation in collaborative learning sessions with your peers, and use of clinical quality measures to improve cardiovascular risk management based on the latest findings from Patient-Centered Outcomes Research. 

EvidenceNOW SW is offered in Colorado through more than 20 local organizations; a practice facilitator will come to your office to work with you on improved workflow, engaging your patients in managing their health, optimize your staff and your EHR, if you have one – not required. This program is a great opportunity to prepare for the evolving world of healthcare based on demonstrating the value of care you deliver; not just activity fee for service). 

The initiative aligns with the Million Hearts campaign with a focus on evidence-based guideline recommendations for aspirin, blood pressure, cholesterol, and smoking cessation. EvidenceNOW SW is a great on-ramp for practices wanting to benefit from grant-funded in-office support to make the changes needed for success in future compensation models that require a different model of care delivery.
