Community health care funding available

The Colorado Health Foundation announces the opening of a new funding opportunity within our Health Care outcome area. In Health Care, our focus is to ensure all Coloradans achieve health with support from a network of primary health care and community services.

Funding Opportunity: Connecting Communities and Care
The Colorado Health Foundation will support as many as 15 Colorado geographically-dispersed communities for as much as $200,000 for two years to facilitate connections between community-based resources and the health care system to improve the health of the community's population. Selected communities will also have access to technical assistance and a peer-to-peer learning collaborative.

This initiative is designed to help communities improve cross-sector collaboration, and is focused on four health conditions: obesity, mental health, diabetes and heart disease. These conditions were identified as top priorities for most of the state through an analysis of the Colorado Department of Public Health and Environment public health assessment data. Funding will support and accelerate existing partnerships working to create community health beyond the clinical setting by linking existing resources and programs between health care and communities.
