Medical Home Community Forum April 14: Quality Care for Colorado Kids

The topic of the April 14 Colorado Medical Home Community Forum is quality care for Colorado kids. Speakers will give an overview of the trends affecting children in Colorado and review a report released by the Colorado Children's Campaign, Ensuring Quality Health Care for Colorado’s Children.

The Colorado Medical Home Initiative is a statewide effort that promotes a patient- and family-centered medical home approach. The initiative hosts community forums every other month at CDPHE. The forums bring together providers, policymakers, community members and families to examine ways to support medical home initiatives in Colorado.
