Mark your calendars for "Treating the Whole Patient: Addressing Social Determinants of Health"

With a growing emphasis on the role of social determinants in health, more efforts are leveraging payment and delivery system reforms as an opportunity to promote better integration of health care and social services. While such innovative models are demonstrating the potential for "bridging" health and health care and using tools to support enhanced team-based care delivery, less is known about how to build financial supports for these models. This webinar will offer insights from two innovative initiatives that are aligning health care and other social services, and how these enhanced models can work under new payment mechanisms.
