Million Hearts in Colorado: The Role of Public Health Nurses in Hypertension Prevention and Control

Prevention and control of hypertension must address both social determinants of health and lifestyle changes, which cannot be managed through clinical care alone. Partnerships between public health and health care are essential to promoting the ABCs and changing the context of health culture. 

The Million Hearts Initiative provides an opportunity for community and public health nurses (PHNs) to contribute to the goal of preventing one million heart attacks and strokes in the next three years. 

The Association of Public Health Nurses is pleased to share with you the 2014 Million Hearts Issue Brief developed in association with ASTHO. The brief describes the current and potential roles of public health nurses in hypertension prevention and control.
