Million Hearts in Colorado: New Tools and Patient Resources on Hypertension

The AHA has created these new tools to assist in the treatment and education of patients:

Controlling Hypertension in Adults: The downloadable pocket guide features the AHA's Hypertension treatment algorithm, developed in partnership with the Centers for Disease Control and American College of Cardiology, and is a critical tool focused on 8 principles for an effective, systemic and team-based formula for the healthcare setting.

Understanding and Managing High Blood Pressure: Available as a downloadable PDF or as an interactive digital workbook, the guide was designed to facilitate dialogue between the patient and provider. The guide provides practical information to educate the patient on the risk factors, treatment and measurement of high blood pressure, as well as empowers the patient to manage their high blood pressure with tips for daily living.
