Million Hearts Colorado: Updated NCQA Patient-Centered Medical Home Standards Raise the Bar

Taking care of your heart is a team effort. Physicians, nurses, community health workers and pharmacists are just some of the members of a client's team that help keep blood pressure, cholesterol and diabetes in control.

The National Committee for Quality Assurance (NCQA) has updated Patient-Centered Medical Home (PCMH) recognition standards, emphasizing the importance of team-based care, establishing it as a “must-pass” criterion for NCQA Recognition. Learn more about NCQA PCMH Recognition.

Other key changes affecting the advantages and requirements of NCQA PCMH Recognition include a new focus on care management for high-need populations, alignment of quality improvement to improve patient experience, cost and clinical quality, sustained transformation and integration of behavioral health. Download the new standards for free and register for a 60-minute webinar from NCQA on April 23 to learn more.

High blood pressure management is a key strategy of the Million Hearts Initiative, an effort to prevent 1 million heart attacks and strokes over five years. Colorado participates in this initiative and you can too. Be one in a million - make your commitment and pledge today.
