Jeffco Seeks Input on Health Plan

Jefferson County Public Health (JCPH), in partnership with the Jefferson County Board of Health, Jefferson County Health Council and numerous community partners, has been seeking input from the community to develop and implement a Community Health Improvement Plan (CHIP). Individuals have been given the opportunity to prioritize the top risk factors affecting their health and the health of their neighborhoods at community engagement meetings held in various locations of Jefferson County. 

JCPH is still collecting input and wants to hear from as many people as possible via an online survey created specifically to gather input from those who may not have been able to make one of the engagement meetings. To give your input, and watch a brief video about the top risk factors in Jefferson County, please follow this link The video and survey will only be available online for two weeks.
