
The champions' practices achieved blood pressure control rates higher than 80 percent for their entire patient populations by using electronic health records to track and monitor patients; sending e-mail and phone reminders to increase medication adherence and encourage healthy lifestyle changes; and teaming up to prioritize high blood pressure control.
Million Hearts is a national public-private initiative led by the Centers for Disease Control and Prevention and the Centers for Medicaid and Medicare Services, with the goal of reducing the number of heart attacks and strokes in the U.S. by one million by 2017.
