Early Childhood Obesity Prevention Professional Development Survey



If you are a health care professional working with families of young children (ages birth – 5 years), pregnant women, or women of reproductive age, please click here to complete a survey.  The purpose of this survey is to gain a better understanding of existing practices to prevent early childhood obesity prevention as well as interest in, and preferences for professional development opportunities. Your responses are extremely important and invaluable to guide early childhood obesity prevention efforts at Colorado Department of Public Health and Environment (CDPHE).
