May Update: Coordinated Chronic Disease State Plan Next Steps


Thank you to the Alamosa, La Junta, Sterling, Denver, Frisco, Durango and Montrose local partners who offered valuable input toward the development of a coordinated chronic disease state framework. 

Approximately 125 stakeholders joined the community forums which included an overview of recent CDPHE health efforts and small group discussions on five key domain areas.  Participants represented various public and private organizations.

FEEDBACK & INPUT
Stakeholder input is welcome throughout the engagement and planning process and can be submitted by visiting the Coordinated Chronic Disease State Plan Feedback & Input Form.

WHAT’S NEXT?
  • A Coordinated Chronic Disease State Advisory meeting will be held on Wednesday, May 30, 2012 from 1 – 5 PM MTN. The team will review and discuss themes heard during the regional forums, and outcomes and strategies to be included in a unifying, statewide coordinated chronic disease framework.   • Stakeholders will have the opportunity to provide feedback on the finalized draft of the state framework.
