COMMUNITY FORUM UPDATE
Four community forums have been held to date in effort to redesign a Coordinated Chronic Disease State Framework. These regional meetings have provided engaged, thoughtful and optimistic discussions around a collective approach to improving the health and wellness of Coloradans. A broad range of stakeholders attended meetings in Montrose, Frisco, Denver, and Sterling including local and state partners, representatives from community and advocacy organizations, and from public policy and higher education. Remaining community forums are listed below – we encourage you to attend!
MEETING PROCESS
The community forum process has allowed participants
to provide valuable input on ways CDPHE can support local efforts to reduce the burden of chronic
disease. Through small group discussions participants have the opportunity to
share their vision for the coordination of the chronic disease state plan and
discuss the following domain areas:

Community Forum can be viewed online at the
CDPHE Prevention Services Division Chronic Disease Livestream Channel. Both
the Chronic Disease State Plan
overview presentations, as well as the recap of the small group sessions to develop strategies to
execute the plan are posted.

WHAT’s NEXT?
The State Advisory Team will provide input on themes and approaches
from community forums. This group who is made up of key statewide
representatives of chronic disease areas will assist with strategizing and
prioritizing the state framework.
