Coordinated Chronic Disease State Plan

Reducing the burden of chronic disease on Colorado’s citizens is a public health priority of the CDPHE’s Prevention Services Division (PSD). 

Although Colorado is a recognized leader in this area, a chronic disease state plan is needed to create a system and linkages among programs and initiatives.

WHY A COORDINATED APPROACH?Creating a state and local system of chronic disease prevention and health promotion starts with a common vision and shared agenda for action. In Colorado, this is outlined in Colorado’s 10 Winnable Battles and PSD's chronic disease Integrated Workplan, which has directed internal chronic disease prevention work at CDPHE for the past three years.

A chronic disease state plan will provide us the opportunity to strategically align all resources dedicated to chronic disease prevention and control and provide direction for state and local, public and private partners in chronic disease prevention.

STAKEHOLDER ENGAGEMENTThe Spark Policy Institute was hired by PSD to engage communities and state-level partners by designing and convening seven community forums with local health departments, their community partners and community members state-wide. The purpose of these meetings is:
  • to develop a shared understanding and vision for a coordinated chronic disease approach in Colorado and
  • to solicit community input on a state plan.
Through these community forums, all participants will better understand local opportunities, successes, obstacles and concerns in implementation of a coordinated chronic disease approach.
