CDPHE PSD is hiring TWO Local Public Health Chronic Disease Coordinators


The Prevention Services Division is hiring two new positions to advance local public health planning and capacity to implement evidence-based strategies to reduce the burden of chronic disease. One position will be housed in the Pueblo City-County Health Department in Pueblo and the other position will be housed in the Weld County Department of Public Health and Environment in Greeley.

This position(s) will serve as the strategic director of approaches to Chronic Disease prevention and control at the local level and manage the development and implementation of Chronic Disease program integration and infrastructure building at the local public health department level. In collaboration with Amendment 35 grant review committees, the State Chronic Disease Leadership team and other chronic disease programs, positions will lead efforts to align programmatic work with a more integrated model of chronic disease prevention and management and focus these efforts on evidence-based practices. The manager will actively participate in all aspects of these activities and process, including participation in all meetings, communications budget monitoring, planning, and evaluation.


Specific Duties
  • build a system for chronic disease infrastructure in the assigned county
  • identify existing systems and infrastructure for chronic disease prevention including major risk factors such as tobacco and obesity
  • develop partnerships to build support and enhance resources for chronic disease prevention and management and integration
  • develop a plan for action to grow the chronic disease infrastructure in coordination with supervisor, community partners, statewide organizations and coalitions
  • manage and coordinate the implementation of the plan and intentionally create linkages between the state and local chronic disease prevention systems
  • maintain content expertise in chronic disease prevention including policy and environmental change, health systems, community clinical linkages and surveillance and epidemiology.
