Job Announcement - Medical Home Policy Coordinator

The Colorado Department of Public Health and Environment's Prevention Services Division is hiring a Medical Home Policy Coordinator in Denver. The position will provide coordination in promoting a medical home approach with a focus on communicating and disseminating information related to medical home policy to local communities, state partners, other professionals, key policy makers and leaders through diverse communication networks and updated technology. The position announcement closes on August 26, 2011.

Goals of this position include:
  • Stimulate change related to medical home systems building through informing or developing policy initiatives that are data driven and based on research. 
  • Monitor advances in the promotion of medical home policies and will facilitate the implementation of such advances at the community level.
  • Utilize the expertise of members to advance the development and implementation of the Medical Home Implementation Team's action plan.
  • Consult and provide expertise on the medical home approach with the media, local public health agencies, other community-based partners and state and national maternal and child health partners, and other highly regarded professionals in public health. 
  • Use up-to-date media techniques to provide web-based content, fact sheets, utilizing and responding to communications on medical home content.
